Brazilian Consulate in Chicago: documents required to renew passport

We have already taught how to renew your Brazilian passport in the USA. However, each of the 10 Brazilian Consulates present in the United States may require certain documents in the renewal process. Therefore, the documents required by the Chicago Consulate are not the same as those requested by other Consulates.

First of all, you need to know that all documents presented must be original. In some cases, when it is not possible to present the original documents, copies certified by a Brazilian notary's office or authenticated by a Notary Public with the appropriate certification from the County Clerk or Register of Deeds are accepted.

See below the list of documents required by the Chicago Consulate:

Documents required at the Chicago Consulate for minors under 18

Notes: it is only possible for one of the parents alone to have the right to request their child's passport if one of the parents is deceased or if one of the parents has sole custody of the minor. In both cases, it is necessary to present recent documents that prove the situation.

The parents' signature on the authorization form does not need to be authenticated if both parents appear at the Consulate. However, it is necessary to check the possibility of carrying out the service in person, as well as scheduling an appointment time.

Documents required at the Chicago Consulate for people over 18 years old

  • Protocol request form printed with 2×2 photo and signature in the indicated places;
  • Current passport (valid or expired less than 2 years ago) – original and copy;
  • Brazilian birth certificate, Brazilian marriage certificate or Identity card (RG) – original and copy;
  • Military document (for men between 18 and 45 years old) – original and copy;
  • certificate of Electoral Discharge (for people aged between 18 and 70);
  • Money orders from USPS in the amount of $120.

Notes: if there is a name change due to marriage/divorce, it is mandatory to present the Brazilian marriage certificate (or consular registration).

If your previous passport has expired for more than 2 years, also present another identification document such as an identity card, consular registration card, professional card or driver's license – original and copy.

If your previous passport was stolen, lost or damaged, you must fill out and sign the loss form and present one of the identification documents mentioned above.
